The fee for our service is based on type and extent of treatment provided. Please let us know your financial concerns before we begin treatment. We require full payment at completion of our services.
Visa, Master Card, American Express, cash, and checks are accepted.
For those patients with insurance, a minimum deposit of $300.00 to $350.00 is required at completion of treatment. We will be happy to submit your insurance claim for you, provided you are able to furnish the information required to complete your forms. Please be aware that your plan is a contract between you and your insurance company and there could be limitations. Once your insurance company has paid, you may receive a partial refund or a bill for the remaining balance.
